Sewing Machine Forms - Due to lockdown we are taking details of beneficiaries on phone every 4th day of the month in the prescribed format which would have basic details of beneficiaries along with contact number. Details will be collected only on 4th day and then lottery would be done on computer (software) on next day. Once lottery is done the selected beneficiaries would be called along with the documents on 10 th of every month to fill the form and submit the required documents.
General Medical Aid (GMA) & High Cost Medical Aid (HCMA) Form issued on 1st, 2nd & 3rd of every month. If either of the dates falls on Sunday, then next day forms will be issued.
